I posted elsewhere for help with the form. Now the form works, but the page design is altered.
Can anyone tell me what's caused this and how I can fix it? Thanks in advance.

I'm not an HTML expert, or even adequate at it, but that code should help.
It is possible that it is too wide and it is forcing the design out of shape, can we see a page without problems?
as to fon_top02.gif a way to stop that repeating is to do it in CSS. find the td elements that should have is and make them class="fon_top" then add
.fon_top {
background: url(images/fon_top02.gif) repeat-x;
}
then it will never repeat downwards, onlt horizontally
